"blue balls" is a slang term, usually used to describe a fake medical conditon... Like many fake medical conditions, this one is very loosely based on a real medical condition, but the causes, effects, and consequences are grossly misstated.
While situations and circumstances that cause vasocongestive pressure and discomfort do (or can) exist, both in males and females, and males can also be subject to priapism, the condition sex-ed aged kids are referring to for the causes they cite when they say "blue balls" - i.e.: "Dude... If you want a girl, and you get a hard on, and you don't get off, you can die!" - doesn't exist.
