What is that thing behind/above Quentyn?

I assume it's neither the view through a fire escape nor somebody sneaking up behind him...

it is. Look at the way the rain is falling, its vertical and spleared out from the middle.

I do believe that most of what is falling on Quentyn is drip water, not fresh rainwater. Do we even want to know what sort of filth that stuff leached through to get to Quentyn's head?
