Do a google on electrostimulation / electrosex. While current units typically use anal probes/electrodes rather than collars, they can cause the same effect... (although not as quickly)
electroejaculation is typically used on men with spinal cord injuries, as it can cause an orgasm with absolutely no mental interaction... electrostimulation / electrosex is designed to cause pleasure, and many units are available (paradise electro, electro-gear, folsom, meo, rimba, electrastim, and erostek are the first few names that come to mind, but there's many others) for this purpose.

Y'know, being found with an electric butt plug up your arse, dead from a power surge is the most embarrasing way to die I can think of, short of being crushed by a rock while shagging a chicken. And I'm sure that for every person who can build one without plugging it into the mains, there's ten who have to make up some very ineresting stories at casualty. I'm sorry, but deliberately electrocuting yourself sounds like a worse idea than using your head as a battering ram.

It's actually a fairly straightforward (no pun intended) job to get a supply with an upper bound on current, and to limit the amount of voltage actually available at an output. (Hell, they wouldn't make plug-in vibrators if it wasn't!) It isn't exactly a dangerous project, although I'd certainly hope anyone constructing such a device had a fair degree of education on the subject.
Because, as you say, dying of cardiac arrest from having an electrical buttplug fibrulate your heart muscles is very easily one of the most embarassing ways to die.

...which is why they're all battery powered. I don't think I've ever seen a unit for sale that ran off AC power.
Most units run off a single 9v battery (and run for quite some time; very little current is used), which even if applied directly to the electrodes wouldn't cause harm. The output is run through a resistor for each pole, keeping the unit from being damaged if the electrodes short together, as well as preventing the possibility of a burn from a short. High-end units with multiple outputs take the further step of transformer isolating each output, to ensure no current flows between output channels, provide further protection, and to increase the maximum output voltage.
(9v isn't enough; the real units have outputs of several times that. transformer-coupled outputs allow for high voltages at low currents with perfect safety. there is no part or combination of parts in the unit that could fail to cause the output to provide any significant current at voltages above battery voltage.)
