Testicular cancer is pretty darn nasty.

Although, it's a little known fact that several organs help to produce semen; any of those could potentially have become a spinerette, and started to produce webbing. This webbing would then potentially block the urethra, requiring frequent catheterization to allow for urination.

Actually, he's never destined to be "chosen". The spider had to choose him right then and there out of everone in the room as the intense radiation was about to kill it. In my opinion that just makes it even more "normal guy with tough breaks" because instead of just randomly getting powers he's randomly getting cosmicy-totemic powers because he was the best option available.
"Yeah, we probably wanted someone else to have all these gifts and who would have dealt with all the things you went through cause they'd have had fewer issues and personal struggles and in the end everyone probably would have been happier off, but the spider was about to die so you were the next best thing.
