I've read all the FAQs on the topic, and I haven't found out how to do it. The new one has been uploaded and approved, but the old one is still there. Is there some way to name the file so that it replaces the old one?

Maybe. I recall this question being asked before but I don't recall the answer that was given, if there was one.
