How do I make a comment box?

Well, after putting it off and putting it off, I think I'm finally ready to install a comment box. Can anybody tell me how best to do that, html-wise? As a side thing, I'd really like it if it could send me an email when somebody posts in the box.

Btw, those of you who do have comment boxes--do you get much trouble from trolls, flame wars, and other such message board nonsense? If you do, do you have any advice on dealing with it? (That's the biggest reason I want the comment box to email me when someone posts; I'd be afraid of some nitwit trolling the early strips, and I wouldn't know about it until I stumble across them.)

If you want a Shoutbox like Lightbringer has, it's very simple -- the Shoutbox site should give you something like this to copy-paste into your .html file:

If you want something more complex, Disqus looks pretty good, although I don't have any experience with it. It seems simple to implement, giving you some code like this to copy-paste:
