Well, as I said, you now add your strength score to get your HP instead of your strength mod. So your HP are low.

Just to note, Second Wind can only be used once per encounter or every five minutes. Otherwise, you need a medic.

In 4e, surges are part of your health meter. As long as you've got some left, 'tis only a flesh wound.
